MSW Social Worker

Badger Hospice, LLC is looking for a Full Time Medical Social Worker to facilitate Psych Social Services to our patients and families in the Waukesha County area. No weekends, nights or holidays! Flexible scheduling options.

Qualifications

Current License in Social Work required

Masters degree in Social Work required

One year of Medical Social Work experience in a health care setting. Hospice and/or home health experience preferred.

Licensed driver with an automobile that is insured and maintained in good working order.

Job Description

1. Assists the physicians and other members of the health care team in understanding significant social/mental stress and/or disorder that exacerbates the symptoms related to terminal illness.

2. Assesses the patient’s social and emotional status in order to estimate the client’s capacity and potential to cope with problems of daily living and assesses the caregiver psychosocial status.

3. Participates in development and revision of plan of care to assist the patient in resolving difficulties, which affect his/her ability to benefit from care provided.

4. Helps patient and family to understand, accept and follow medical recommendations.

5. Assesses the patient/family psychosocial status, potential risk of suicide and/or potential for abuse or neglect.

6. Assesses emotional factors related to terminal illness.

7. Provides social services including short-term individual counseling, crisis intervention, assistance in providing information and preparation of advance directives, funeral planning and issues involving transfer of fiscal, legal and health care responsibility.

8. Identifies family dynamics, communication patterns and involves the family/caregiver’s ability to access them.

10. Identifies and utilizes appropriate community resources and assesses patient/family/caregiver’s ability to access them.

11. Evaluates patient/family response to psychosocial interventions.

12. Assesses caregiver’s ability to function adequately.

13. Assesses special needs related to cultural diversity including communication, space, role of family members and special traditions.

16. Identifies obstacles to compliance and assists in understanding goals and intervention.

17. Identifies support systems available to reduce stress and facilitate coping with end-of-life care.

18. Communicates psychosocial information to inpatient facility when level of care is changed.

19. Assists family in assessing financial resources when appropriate.

20. Attends team meetings, and maintains effective communication with other team members.

21. Prepares clinical and progress notes.

22. Assesses bereavement needs.

23. Is involved in the QAPI program.

24. Performs other related tasks as required.

Job Type: Part-time

Pay: $50,000.00 - $65,000.00 per year
