YOUTH CONTINUUM, INC is seeking a dedicated and passionate RN/ LPN!

The perfect candidate will be responsible for coordinating and supporting the health care needs of youth in the program according to DCF and Academy of Pediatrics Guidelines, working with program staff to ensure identified needs are being appropriately addressed. Provide oversight regarding all DCF licensing and COA requirements to ensure compliance with applicable regulations. Supervise the medication administration program, including assisting with policy development, completing staff training, and providing education in related health topics. This is an on-site position with required weekly on-call responsibilities.

RESPONSIBILITIES: Essential Duties and Responsibilities include the following. Other duties may be assigned.

  • Maintain agency medication administration policy and procedure manual.
  • Work with the Medical Director, community providers and consulting psychiatrists to ensure all medication orders are current and that all medication supplies are adequate.
  • Supervise all aspects of administration of medication and be available to consult with staff regarding medication questions, concerns about side-effects and/or appropriateness of medications as prescribed. Oversight on all medication trainings, DCF updates and on-site testing.
  • Work with Program Director to ensure all clinical records maintain appropriate health records.
  • Perform job duties consistent with nursing and license standards. Maintain best practices for care according to DCF set standards. Understand trauma informed care.
  • Evaluate the health care needs of all clients; participate as requested during intake and admission procedures.
  • Conduct medical assessments of minor youth in shelter within 72 hours of admission.
  • Evaluate the physical condition of clients following abuse reports/allegations. Contribute to the service plans for each child in the areas of health care including nutritional health.
  • Work closely with the child psychiatrist and attend all clinical and treatment meetings for each client.
  • Conduct health-related groups for the clients.
  • Keep Program Director and staff appraised of any physical or health care issues which may affect the health and/or safety of clients or staff.
  • Coordinate medical appointments as needed.
  • Take direct charge during medical emergencies if on-site; on call for medical emergencies when not on site.
  • Arrange for medication follow through and monitor the effectiveness or adverse effects of the medication in collaboration with staff; maintain DCF protocols for psychotropic medication monitoring.
  • Provide basic training and education to staff regarding health/medical issues, procedures, first aid and universal precautions.
  • Ensure medication administration and general health standards meet or exceed COA and DCF requirements.
  • Liaison with community health care providers as needed.

CERTIFICATES, LICENSES, REGISTRATIONS, and OTHER REQUIREMENTS:

  • A current/ active CT-LPN or RN license
  • First Aid/CPR Certification
  • A current CT Driver’s License, registration and insurance
  • Pass and maintain a clear DCF abuse and neglect record
  • Pass and maintain a clear criminal record
  • Maintain malpractice insurance
  • Initial TB screen
