RN Case Manager - Home Health

We are a growing healthcare provider based in Ohio, currently seeking a dedicated and dynamic Registered Nurse (RN) for the position of Case Manager in Home Health. Position Overview:

We are looking for an adaptable and dynamic RN to join our team on a full-time basis. This role presents an exciting opportunity to contribute to the growth of our organization. While previous experience in home health is preferred, we are open to candidates without this background and are committed to providing comprehensive training.

Responsibilities:

• Conduct thorough patient assessments and develop personalized care plans.
• Manage and coordinate the care of patients in a home health setting.
• Collaborate with the interdisciplinary team to ensure optimal patient outcomes.
• Provide education to patients and families on managing health conditions and promoting wellness.
• Demonstrate effective communication with team members, patients, and their families.

Qualifications:

• Current RN license in the state of Ohio.
• Ability to be adaptable and dynamic in a growing position.
• Previous experience in home health is preferred but not mandatory; training will be provided.
• Experience with start-up companies is a plus.
• Strong communication skills, with the ability to prioritize tasks and address important issues effectively.

Compensation and Benefits:

• Competitive salary in the range of $70,000 to $75,000 per year.
