-Continuum Home Health Care is seeking a compassionate and dedicated Personal Care Assistant to provide experienced caregiving skills to support and assist individuals in their daily activities between the hours of 12pm to 8pm. As a Personal Care Assistant, you will play a crucial role in ensuring the well-being and comfort of our clients. Responsibilities

- Assist clients with personal hygiene tasks, such as bathing, grooming, and dressing

- Provide assistance with mobility and transfers
- Administer medication reminders as directed by healthcare professionals
- Prepare and serve meals according to dietary needs and restrictions
- Perform light housekeeping duties, including cleaning and laundry
- Accompany clients to appointments or social outings
- Engage in meaningful conversations and provide emotional support
- Monitor and report any changes in clients' physical or mental condition

Qualification

- High school diploma or equivalent
- Previous experience as a Personal Care Assistant or similar role is preferred but not required
- Compassionate and patient demeanor
- Excellent communication skills
- Ability to follow instructions and work independently
- Reliable transportation to travel to clients' homes
- Flexibility to work various shifts, including weekends and holidays
