Diversity Alliance is seeking a dedicated and organized Section 28 Care Coordinator to join our team!

The ideal candidate will play a crucial role in writing treatment plans for clients, supervising BHP's, collecting and analyzing goal data, completing CSR's and 90 day reviews. This is a part-time position between 9-6:30 Mon-Fri with the flexibility to grow into full-time with caseload expansion.

Duties

• Complete continued stay reviews every 180 days.
• Take statistics on client goals and analyze data.
• Complete comprehensive assessments annually with families.
• Write treatment plans and create progress notes for BHP's.
• Supervise a team of BHP's on a monthly basis.

Experience

• Must have a bachelor's degree in social work/psychology or related field.
• Must have one year experience working with children with cognitive and behavioral support needs.

Pay: $26.00 - $29.00 per hour

Expected hours: 10 – 25 per week
