The Palliative Care Social Worker will collaborate closely with other service lines within the organization to engage community-dwelling elders in palliative care. The right candidate will possess strong clinical skills and a passion for collaboration with team members from multiple disciplines. This position includes participation in development of dynamic and growing palliative care program for an organization that values innovation, leadership and continuity of care.

Responsibilities:
  • Offers introductory education to patients and families about the palliative care specialty and palliative care services offered by HSL.
  • Provides direct service including in-person and virtual visits
  • Develops comprehensive bio-psychosocial assessments specific to patients living w/ serious illness.
  • Assesses patient/family needs regarding social determinants of health, offers appropriate resources and referrals as needed
  • Attends interdisciplinary team meetings to review active cases
  • Promotes discussion between the patient, family, and medical team, in order to facilitate understanding of the patient’s medical condition and goals of care
  • Establishes clear, patient-centered care plans
  • Accurately documents observations, interventions and evaluations pertaining to patient care in a timely manner
  • Coordinates and facilitates family meetings
  • Assists patients and families transition from palliative care to hospice when appropriate, providing continuity of care
  • Provides formal and informal educational opportunities for the clinical staff on current aspects of palliative care
  • Assists with supporting Skilled Home Care patients as needed
Required Qualifications:
  • Licensed as a MSW in Massachusetts pursuant to M.G.L.c.112 § 131 under the licensure categories of Licensed Independent Practitioner of Clinical Social Work, or Licensed Certified Social Worker or Licensed Social Worker
  • Familiarity with Jewish life, culture, and religion preferred
  • A minimum of one year of social work experience in a health care setting required
  • Access to reliable transportation to patient's home/facility, may be in multiple locations in a workday
  • Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with patients, families, and colleagues.
