St. Luke's Home

Located in Springfield’s beautiful historic district, Saint Luke’s Home is a real home for its residents. The grounds include lovely gardens that the residents help to maintain.

Inside, all rooms are private, and residents may decorate them with personal items and furniture, or use furnishings we provide. A spacious activity area and large dining room provide areas for socializing with fellow residents, and a serene chapel offers a quiet space and time for devotion and reflection.

With its urban location, Saint Luke’s home is within walking distance of libraries, museums, churches, parks and shops – enabling residents to take advantage of the many resources the city has to offer.

Demonstrates, promotes and integrates the Trinity Health Senior Communities Mission and core values by:

Demonstrates, promotes and integrates the mission of the facility, the mission of the Trinity Health Senior Communities and its core values (including H.E.A.L.) in daily work activities.

1. Promotes optimal well being of residents including; personal care and oral hygiene, medication administration and treatments, hydration and nutrition, exercise and socialization. Obtains vital signs, weights, and specimens.

2. May serve as charge person for a shift; oversees nursing support and care as delivered by others, fulfills routine nursing office tasks i.e.; order transcription, ordering and receiving medications, responding to emergencies, etc.

3. Communicates orally and in writing with residents, co-workers, families, and physicians.

4. Documents activities in accordance with facility procedures in a timely fashion. Notifies Administrator and/or DON of problems, concerns, and safety issues.

5. Follows all safety, security, infection control, quality and hazardous materials policies and procedures. Performs all tasks to ensure resident and personal safety, protection of co-workers and facility property.

6. Customer satisfaction and a focus on quality are evidenced in all aspects of work. Promotes and supports the philosophy, goals, and objectives of THSC.

7. Attends all regulation and THSC mandated in-services; follows mandated abuse/neglect reporting, and maintains confidentiality at all times.


REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

1. Must be at least 21 years of age, have received a high school diploma, have the ability to make mature and accurate judgments and be able to communicate orally and in writing in English (Reference CMR 150-001).

2. Ability to provide personal care with respect for an individual’s privacy and dignity. This skill is usually gained through a HHA or CNA course, studies in nursing or a related field, or caring for a friend or family member with disabilities.

3. Interpersonal skills necessary to relate to persons of varying age, culture, religion, and/or ability.

4. Ability to remain physically active for the duration of the assigned/scheduled shift. Will bend, stoop, lift, stand, walk, and carry supplies in the completion of the assignment. In emergency situations may be required to assist in lifting a person who has fallen.


WORKING CONDITIONS:

1. To minimize risk of illness must understand and practice standard precaution and practice proper body mechanics and Tag Out/Lock Out procedures.

2. Will work rotating weekends and days off, some holidays, and occasional shifts other than usual assignment.

3. Work environment is usually pleasant: works in a team relationship with employees from all departments, and with frequent feedback on performance.


REPORTING RELATIONSHIPS:

1. Responsible to the charge person for daily assignment; reports administratively to the Director, Nursing or Clinical Administrator.

2. Has responsibility for leading others when serving as charge person.

3. May assist in new employee training/orientation.
