Duties will include but are not limited to the following:
- Assist client and/or their guardian with re-determination of financial eligibility as required by the DHHR office in the county where the client lives.
- Provide linkage and referral to community resources.
- Monitor to ensure that the member's health and safety needs are being met.
- Meet with clients monthly at their residence to verify that services are being delivered in a safe environment.
- Meet with clients in non-residential settings, including facility-based day program, supported employment sites, and volunteer locations.
- Comply with all company/state/federal reporting requirements.
- Support clients as necessary to convene and conduct IDT meetings and notify IDT members 30 days in advance of meeting.
- Act as an advocate for clients.
- Communicate with other service providers on the IDT to allow for continuity of services.
- Coordinate and develop client Individual Program Plans (IPP) annually and review at least every 90 days unless otherwise specified in the plan.
- Coordinate critical juncture and discharge/transfer meetings and services.
- Interface with required agencies on behalf of clients in regard to the assessment process, and the purchase of services/budget development.
- Send copies of IPP to the IDT members within 14 days.
- Present proposed restrictive measures to the Human Rights Committee.
Qualifications:
- Bachelor's degree in a human services field or nursing field.
- Valid driver's license.
- Must have two years of experience with case management or related discipline
- Must be able to communicate both verbally and in writing.
