Job Description:

The Diabetes Health Navigator (DHN) is a patient care coordinator who helps connect patients with appropriate treatment and services to address problems related to their Diabetes. Diabetes Health Navigators conduct initial brief assessments, introduce patients to Diabetes programs and services, expedite appointments with appropriate providers, serve as the primary coach for their patients and maintain ongoing contact with their panel. They also assist with access to other services such as financial counseling, primary care and social services. The DHN proactively builds and cultivates a network of service providers within the John Muir Health system as well as with various private and public Diabetes resources throughout the community.

Education:

  • High School Diploma required

Experience:

  • Two (2) years’ work experience required

  • Two (2) years’ Diabetes Experience preferred

Certifications/Licensures:

  • BLS Basic Life Support - American Heart Association - John Muir Health – Required within 60 days of hire and every 24 months (or within 30 days of return from leave of absence)

Skills:

  • Excellent written and verbal communication skills across organizational levels

  • Teamwork oriented and skilled at building constructive and effective professional relationships within JMH as well as community partners

  • Nonjudgmental, energetic and positive approach to assisting patients with Diabetes

  • Fluency in Spanish strongly preferred

  • EPIC preferred (or expected training upon hire and demonstrated proficiency following)

  • Knowledge of computer applications such as Microsoft Word, Excel, and outlook strongly preferred

  • Ability to apply critical thinking skills, ability to multi-task and manage time and work assignments independently

  • Possess the ability to work independently as well as part of a team

  • Ability to work respectfully and creatively with patients of diverse functional ability, social, economic and cultural backgrounds to support both patient autonomy and client safety

  • Sensitivity to diverse religious and cultural practices and values. Ability to work cooperatively and collaboratively with staff at all levels across the organization

Work Shift:

08.0 - 08:00 - 16:30 No Waive (United States of America)

Pay Range:

$30.38 - $41.02 Hourly

Offer amounts are based on demonstrated/relevant experience and/or licensure.

Note: Positions at JMH which are exempt (not eligible for overtime) under the level of Manager are listed as hourly for compensation purposes on this posting. The work shift will contain the word ‘exempt’ on it.
