Description:

GENERAL SUMMARY AND PURPOSE:

Coordinates access to available services provided through the Community Services Department including, but not limited to, lodging meals, and transportation assistance, wayfinding, and Cultural and Language Appropriate Services.

Acts as a liaison between a wide variety of clinical and non-clinical staff, physicians, contractors, patients and families in order to accomplish effective, timely delivery of services to patients, families, customers and employees.

SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE:

Bachelor’s degree in healthcare, business, human services or social sciences such as psychology or social required or minimum of 2 to 3 years of experience in health care or hospitality in lieu of a degree.

This is an excellent position for one who prefers a three-day work week. The hours will be:

Saturday & Sunday's 9:00am-5:30pm and Monday's 7:30-4:00pm

ESSENTIAL FUNCTIONS:

Knows, understands, incorporates, and demonstrates the Organization's Mission, Vision, and Values in behaviors, practices, and decisions. Coordinates the provision of Culture and Language Appropriate Services (CLAS) in accordance with Federal and other regulatory agency guidelines, including but not limited to the Federal CLAS standards, JCAHO standards, and CMS standards as regulated by the Office for Civil Rights.

This includes the provision of interpreters, written translation services, and assistive devices for Limited English Proficient (LEP), Deaf and Hearing-Impaired patients, families, customers, and employees. Serves as a resource for medical staff and care providers in facilitating the delivery of CLAS across the continuum of care to a diverse patient population. Works closely with community refugee relocation organizations, case managers, and other community agencies to facilitate Language-appropriate access.

Demonstrates a high level of proficiency in understanding the complex cultural, socio-economic and political issues surrounding access to, and delivery of, healthcare to patients from a wide range of demographic backgrounds. Recruits, schedules and supervises Sign and Language interpreters to meet the needs of LEP/Deaf and Hearing-Impaired customers in accordance with National language provision standards. Develops and maintains other associated Community Services department databases, including schedules, billing and reimbursement records, compliance and accountability logs.

Coordinates services available to patients and families within the Access Program and the Community Services Department. These services include but are not limited to, lodging and transportation assistance, meal assistance, and assistance with durable medical equipment. Acts as a liaison between staff, vendors, providers and contractors to coordinate and meet the needs of patients, families, customers and employees.

Exhibits advanced problem solving and critical thinking skills to meet the varied physical, emotional, cultural and spiritual needs of our patients and customers. Patient and customer needs outside of the scope of the department may require facilitating access to the services of other departments.

Excellent customer service skills including listening, critical thinking and complex problem-solving abilities as well as attention to detail. Demonstrated ability to coordinate effectively in an organized, professional, principled, and empathetic manner, and able to work with a wide variety of people under adverse conditions.
