Community Programs Coordinator

Organization Description: A comprehensive health and human service agency dedicated to strengthening the emotional and social well-being of individuals and families through accessible, affordable mental health counseling and social service programs.

Position Overview: Reporting to the Executive Director, the Case Management Coordinator is responsible for NFSB’s Case Management Services. The Case Management Services provided include intake, assessment, service planning, application for NJ Save and other benefit programs, monitoring & evaluation. The position primarily supports older adults and individuals receiving or eligible for SSDI with programs that can assist in lowering the cost of prescriptions, Medicare, utilities, and hearing aids, and screen applicants for additional help with utilities, food, and prescriptions through State and Federal programs. However, the program also supports income eligible individuals and families in accessing public assistance and eligible benefits.

Hours: 25 hours/week, Exempt; Flexible schedule possible Monday-Friday between 9am – 4pm

Major Duties:

· Outreach, promote and provide comprehensive case management services at the NFSB agency site and in the community at contracted locations.

· Case Management services provided include intake, assessment, service planning, application for NJ Save and other benefit programs, monitoring & evaluation.

· Maintain a minimum caseload of forty clients.

· Maintain proper client records for all services rendered in compliance with NFSB’s Case Management Manual, and other rules, regulations, and policies.

· Connect older adults and individuals receiving or eligible for SSDI with programs that can assist in lowering the cost of prescriptions, Medicare, utilities, and hearing aids, and screen applicants for additional help with utilities, food, and prescriptions through State and Federal programs.

· Assist clients in navigating and obtaining Supplemental Nutritional Assistance Program (SNAP), Low Income Home Energy Assistance Program(LIHEAP)/ Universal Service Fund (USF), Emergency Broadband Benefit (EBB) to support Internet connection, obtaining Medicaid insurance, New Jersey Hearing Aid Program (NJHAP), medical cost assistance through programs such as Medicare Savings Program (MSP), Medicare Part D, Pharmaceutical Assistance to the Aged and Disabled (PAAD), and Senior Gold Prescription Discount Program.

·Collaborate with NFSB Mental Health, Food Pantry and Social Service staff and make referrals to the Food Pantry and Mental Health Services as needed.

· Identify, cultivate, and sustain relationships with community resource and referral partners.

· Provide weekly supervision and ongoing training to BA level interns.

·Complete service documentation and reporting utilizing electronic health records system.

·Evaluate participant’s overall progress and report to funders/referral sources as requested.

· Participate in required staff meetings, supervision, training, and orientation as required by funding and accreditation.

·Other duties as identified by the Executive Director.

Qualifications:

· Advanced degree in social work or a comparable human service field and a minimum of two years’ experience in direct services or case management required.

· Licensure or certificate in case management preferred.

· LSW license a plus but not required.

· Bilingual in Spanish a plus but not required.

· Must have the ability to coordinate services as part of a team; good at linking service recipients and making referrals to community services; and knowledge of public assistance programs, eligibility requirements, and benefits.

·Must have the ability to work autonomously and with initiative; manage multiple priorities simultaneously; organize time efficiently; be adept at problem-solving; work under pressure; be detail oriented; and possess sound judgment and discretion when handling confidential information.

·Experience working with adults 65 and older and individuals eligible for SSDI a plus.

·Excellent interpersonal and communication skills (both written and verbal) are required.

·Proficiency in MS Office.
