Care Transition Manager / LMSW or LCSW / PRN

Work hours: Part Time, 24 hours weekly, Days during the week and weekend days

Here’s What You Need
· Master's Degree Social Work is required. (Individuals hired as a CTSW prior to May 11, 2017 will be grandfathered to the CTSW position with BSW, at the entity they were employed at on May 11, 2017)
· Three (3) years in hospital/medical social work is highly preferred
· One (1) year of discharge planning/care management is preferred
· LMSW - Licensed Master Social Worker or
· LCSW - Licensed Clinical Social Worker is required upon hire.
· CPR - Cardiopulmonary Resuscitation is required upon hire.
· ACM - Accredited Case Manager is preferred upon hire or
· CCM - Certified Case Manager is preferred upon hire
· Other ANCC certifications also preferred
What You Will Do
· Facilitate care conferences for complex transitions, placement and palliative care needs. Serve as a point of contact for all identified stakeholders.
· Proactively identify patients who no longer meet continued stay criteria and communicates with the physician team. Schedule PCP, specialist or clinic follow up appointments for patients.
· Participate in Joint Commission and other survey readiness activities and clinical review staff requirements and communications.
