Job Description

Provide integrated behavioral health services to patients currently receiving primary care services. The Mental Health Specialist position is the primary interface for patients to access services such as peer/family counseling and support, screening, assessment, brief treatment, consultative services, education and engagement, linkages, outreach and advocacy. As a licensed clinician, license eligible or BBS registered mental health intern, the Behaviorist will provide brief consultations of motivational interviewing and facilitate linkage and referrals to community and higher level care providers for adult patients. Working from an interdisciplinary approach and in collaboration with the staff Psychiatrist, the Behaviorist also provides orientation, consultation, and ongoing training to pertinent primary care staff as needed in order to strengthen the relationship between primary care and behavioral health services.

Founded in 1921, Straub Medical Center includes a 159-bed hospital in Honolulu, a network of neighborhood clinics and a visiting specialist program that reaches throughout the state of Hawai'i. With over 200 physicians who are leaders in their fields, Straub provides its patients with diagnoses and treatments for more than 32 different medical specialties, including bone and joint, heart, cancer, endocrinology/diabetes, family medicine, gastroenterology, geriatric medicine, internal medicine, vascular and urology. Straub is home to the Pacific Region’s only multidisciplinary burn treatment center. The hospital consistently brings new technologies and innovative medical practices to Hawai'i, such as minimally invasive cardiac surgery and total joint replacement.

Comprehensive Primary Care Plus (CPC+) is a national advanced primary care medical home model that aims to strengthen primary care through regionally-based multi-payer payment reform and care delivery transformation. The use of team-based care features uniquely skilled individuals working together, in new ways, to meet the health care needs of patients in an efficient, comprehensive, and sustainable manner. Successful population management relies on a high-functioning team that shares a firm vision of a healthier patient who is empowered to better manage his or her own health, with the help of Straub's Care Team. These physician-led teams are aware of the patient’s medical and social history, and are equipped to attend to patient needs in a variety of ways. Minimum Qualifications: Master’s Degree in Social Work. Current Hawai‘i Clinical Social Work (LCSW) license. Current American Heart Association Basic Life Support (BLS) card. Professional knowledge of loss, illness, and disability issues.

Preferred Qualifications: Two (2) years of experience in a Licensed Clinical Social Worker role.
