Avera is currently seeking a BHS Patient and Family Navigator!


Position Highlights:

Job Summary

The Navigator will serve as a patient liaison to help patients and family members “navigate” the complex healthcare systems. This position will work with a multidisciplinary team providing improved comprehensive behavioral health services to patients. This position requires expertise to identify and implement improvement processes, and the ability to design, direct and implement teaching plans. Internal and external contacts include nursing staff and management, physicians, patients, families, community agencies, and professional peers.

The ultimate goal of the program is to provide care that is coordinated and navigated along an agreed care path with all information available to patients and families as needed, and thus, minimizing barriers along the full continuum of care.

Education and/or Experience

  • Bachelor’s degree in human services filed such as social work, psychology, behavioral sciences, or nursing required.
  • One year mental health experience required
  • Community Healthcare Worker (CHW) is preferred

Work Schedule

  • Day, Evening and Weekend Hours (7 days per week, with overnight potential)

Pay

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

  • $19.50 - $29.50 per hour
